Module 2: Personal Care, Health, Social Skills, and Safety
Learn about the medical field and health industry with these worksheets. For item # 1 in the PDF, Muslim students can alsochoose to explore influential Muslims in the field of medicine, such as Ibn Sina (aka Avicenna)

14 page packet. Includes tips for setting up a physical fitness program, exercise log, medical history form, activities to help children explore the importance of physical fitness. They are Boy Scouts worksheets, but you can just adapt for home use as needed.

Middle School and Up
(Day 1)
Have student complete the Health Care Skills assessment (pp. 12-13) of Module 2 (PAYA Workbook). You could make this a discussion and do it orally with student.
On Medical notebooking pages, have student write a journal entry discussing areas he/she was familiar with and areas that student would like to strengthen knowledge of and/or have student fill out goal flowchart, page 15 of notebook.
Go over page 16 with student. Have student record responses in notebook/notebooking pages.
Day 2 - ?: Nutrition
Vocabulary: Have student define the following words:
preservatives (define and research and find 3 things used as preservatives past/present).
sodium
calorie
saturated fats
cholesterol
monosaturated fats
polyunsaturated fats
Discuss pages 19-22 with student regarding nutrition. Be sure to go over the quiz and the answers.
Activity 1:
Look at 7 or more food packages in the home and find/record their sodium contents in milligrams. If not given in milligrams, convert.
Compare the amounts. Were you surprised that some foods had more sodium than others or was it what you suspected?
Make a chart or poster showing your findings.
Activity 2:
Make a mini poster (for lapbook/notebook) showing different forms that sugar comes in in products (like the illustration). Use actual pictures where able. Somewhere on the poster, define the define all the names of the sugar forms.
Activity 3:
Make a 3 column chart showing the difference between monosaturated, polyunsaturated and saturated fats. Under each type of fat, list examples/provide illustrations of examples.
Have student make a "Did you know?" and copy the information given about cholesterol.
Optional: Have student research arteriosclerosis and heart attacks/strokes caused by fats. Present the findings in written form (paragraph or other form such as a brochure, news article, etc).
Have student create a food posters/collages based upon the explanation of the answers to the quiz. (There are different topics covered so several visual representations would be nice).
Begin the food diary. Follow instructions in workbook. (Pages 22-24)
After one week of recording, discuss/complete page 25-26 (assessment of food diary).

Optional: Read about Eating Disorders on page 26-27.
Assign page 29 (oral/written)
Have student answer questionnaire on page 30 and assessment personal care needs on page 31. Record in notebook.
Work with student to complete the family medical history form/questionnaire on page 32, plus the exercise at the bottom of the page. There is a link to a more detailed form at TJ's Orderly Muslim Home site. You may want to fill this one out and keep in your records/household notebook.
Have student fill out the sample Health questionnaire form on page 33-38. This form is similar to what might be asked when visiting a new doctor or at a clinic. Discuss with student any terms he/she does not know. Student may want to make a list in notebook of terms not known.
Taking Care of Yourself -Prescription Medications/Minor Illnesses/Emergencies pp.39-43.
General Survival Skills
Health Risks - Smoking Pages 48-53. Do the activities and find Islamic proof as to whether or not smoking is permissible.
Health Risks - Drugs
Health Risks - Alcohol.
Islamically this is haram, but student should still be knowledge about alcohol and its affects (page 70-71).
Base study/activities upon Medicine worksheets/activities (offsite) (PDF) (If we get to this this year, I'll provide an outline of what we did/had planned, insha Allah.
